This position works closely with all the department heads in order to provide a continued quality of care to the residents. The position requires an understanding and a commitment to the principals and values of the Community, including experience related to the progression of dementia. The Resident Care Coordinator must be able to be flexible and creative, but also have good time management skills and the ability to lead by setting a good example. This position also requires a person who can provide training and follow up to ensure the continuity and quality of care continues. Essential Duties and Responsibilities General management responsibilities include, but are not limited to overseeing and accepting the final accountability for the following either directly or through designated staff: - Become proficient in the service plan program, gathering the necessary information on the care needs of the resident prior to the service plan meeting and then being prepared to present any changes to the service plan to the resident and family members. - Responsible for the over-site of ordering supplies and medical equipment as needed. - Assist the Wellness Director with the tracking of incident reports and investigations. - Ensure that staffing levels remain within established and approved budgetary and staffing guidelines. - Fill in as a Medication Tech and Caregiver as needed. Required Experience: - High school diploma or equivalent or minimum education required per state. - Combined experience in supervising, training and mentoring staff as well as medication administration and care giving experience in a senior living environment is preferred. - Training available for qualified candidates. - Must be able to speak English proficiently. Required Experience: - A current nursing license in the state of employment without restrictions. - Prefer prior nursing experience in a long term care setting. - Must be able to speak English proficiently. - Proficient in logging electronic medical records and other everyday computer skills. - All candidates must be able to pass a fingerprint/criminal background check and a comprehensive reference check. This includes creating the monthly activity calendar, arranging outside entertainment, assessing resident preferences and guiding programs to meet their needs.  This position must initiate and maintain effective communication with Community team members on all appropriate matters. This position requires creativity, flexibility, time management skills, and a knowledge base of the geriatric population who may have complex medical needs. It also requires the ability to provide health services training to other employees of the Community, and to oversee the medication administration program. Essential Duties and Responsibilities General management responsibilities include, but are not limited to overseeing and accepting the final accountability for the following either directly or through designated staff: - Responsible for fulfilling daily, weekly, and monthly job checklist duties and other duties as needed. - Assist the Executive Director to ensure that policies, procedures, and state regulations are being followed. - Conduct internal quality assurance departmental audits. - Assess the health conditions and needs of the residents upon move in and when a change of condition indicates assessment. - In conjunction with the Program Manager/Executive Director, establish the appropriate level of care for the resident, based on their care needs. - Ensure the resident safety needs are met, utilizing specific assessment and evaluation tools. - Ensure routine resident assessments are met on a quarterly basis. - Participate in the development of the Plan of Care; Attend the Plan of Care meetings as indicated by Community policy or the resident’s medical condition. - Serve as a liaison between the Community, family and resident when needed. - Perform medication room and medication cart audits as needed. - Monitor the 24-hour communication and alert system for compliance; Appropriately and timely assess residents who have been put on alert status; Remove residents from alert after appropriate assessment of their condition. - Review all physician’s transcriptions, progress notes, discharge orders, move in orders, and hospital notes; update as necessary every 90 days. - Investigate incidents and behavior changes that require a License Nurse assessment; Document all incidents and changes in the resident’s progress notes. Competitive wages. Required Experience: - High school diploma or equivalent or minimum education required per state, higher education preferred in a related field - A current RN license in the state of employment. - Prefer a minimum of one (1) year prior nursing experience in a Senior Living Community or related business. - Must be able to speak English proficiently. - Training available for qualified candidates.
