Required Experience: - A current nursing license in the state of employment without restrictions. - Prefer prior nursing experience in a long term care setting. - Must be able to speak English proficiently. - Proficient in logging electronic medical records and other everyday computer skills. - All candidates must be able to pass a fingerprint/criminal background check and a comprehensive reference check. If you are a motivated individual with a passion for senior care, we want to meet you.  Westwind Memory Care located in Santa Cruz, CA is seeking a passionate and strong customer-oriented Community Licensed Nurse.  The Community Licensed Nurse assists with ensuring the quality of care of all residents. This position requires creativity, flexibility, time management skills, and a knowledge base of the geriatric population who may have complex medical needs. The position will provide assistance to the Wellness Director.   Essential Duties and Responsibilities General management responsibilities include, but are not limited to overseeing and accepting the final accountability for the following either directly or through designated staff: - Responsible for fulfilling daily, weekly and monthly job checklist duties and other duties as needed. - Assess the health conditions and needs of the residents upon move in and when a change of condition indicates assessment. - In conjunction with the Wellness Director, establish the appropriate level of care for the resident, based on their care needs. - Ensure the resident safety needs are met, utilizing specific assessment and evaluation tools. - Become proficient at gathering and in-putting information into an electronic care/service plan system; Implement interim service plans as needed. - Ensure staff becomes familiar with changes in the resident’s care needs. - Serve as a liaison between the Community, family and resident when needed. - Communicate and coordinate with case managers, guardians, family members or regulators when resident’s safety is at risk. - Monitor the 24-hour communication and alert system for compliance; Appropriately and timely assess residents who have been put on alert status; Remove residents from alert after appropriate assessment of their condition. - Review all physician’s transcriptions, progress notes, discharge orders, move in orders, and hospital notes; update as necessary every 90 days. - Investigate incidents and behavior changes that require a License Nurse assessment; Document all incidents and changes in the resident’s progress notes. - Become knowledgeable of all state regulations governing assisted living communities. - Assist in writing service plans with the Resident Care Coordinator. - Perform nursing assessments as instructed by the Wellness Director.   Required Experience: - High school diploma or equivalent or minimum education required per state, higher education preferred in a related field - A current license in the state of employment. - Prefer a minimum of one (1) year prior nursing experience in a Senior Living Community or related business. - Must be able to speak English proficiently. - Training available for qualified candidates.
